The new Capay Valley Health and Community Center (CVHCC) will host multiple services in one location for people living in Esparto and throughout the Capay Valley. The new 28,000 sq. ft. building will be a hub for health and social services and will allow the people on the western side of Yolo County to access services.
Yocha Dehe Wintun Nation is dedicated to ensuring the economic benefits of the Tribe's gaming operations are felt locally, in communities within its ancestral territories. Located in the Capay Valley, tribal citizens remember well the difficulties and challenges of being extremely impoverished and living without basic necessities. Today, the heart of the Tribe remains in helping others escape the stranglehold of poverty.
RISE Inc. provides and delivers seamless services from pre-school education through senior citizen recreation and multiple resource programs. RISE offers after school programs funded through the California Department of Education (CDE), Workforce Investment Programs for youth (WIOA), emergency food, clothing, and a variety of resource case management and referral services to all community members. In its 30-year history, RISE has also served as the conduit for innovative programs, service initiatives and increased opportunities into the region.
WHF, also a non-profit, currently provides services out of a facility in nearby Winters. WHF will provide primary care, behavioral health, dental services and pharmacy services to the community. The medical care team will provide the full spectrum of primary care services including perinatal, well and sick child, adolescent health, and geriatric health, as well as a wide range of preventive health services. Mental health providers work alongside the primary care team to provide primary mental health services including prevention, screening, diagnosis, treatment, and follow-up of common mental health disorders. A full range of primary dental and optometry services will be offered to all health center patients.
Brown Construction is an organization that is dedicated first and foremost to being a member of the community with the driving mindset that success comes from enhancing the communities in which people work and live. Through its “Build it Forward” program, Brown Construction waives all profits on one community-focused project every year. The projects vary, but the goal is the same: to build facilities that create a stronger community. Brown Construction has committed to building the Capay Valley Health and Community Center as its next “Build it Forward” project.
This project is projected to break ground in mid 2021 and be completed in the fall of 2022.
The goal of this project is to create a community hub where multiple organizations will be located and provide a partnership between health, social, and county services.
The majority of services will be offered by two main providers, RISE, a non-profit 501(c)3 in operation for more than 32 years and Winters Healthcare Foundation (WHF), a Federally Qualified Health Center, founded in 2000. In addition, Yolo County will have a base to provide human services, political representation, and law enforcement access for rural citizens.
This model will increase access to timely care, streamlined referrals and increased communication between organizations. A relationship-based model of care is at the center of the organization culture.
